To ensure that your first visit with us goes as smoothly as possible, you can review these key things to know.
Things to Know
Please bring your films to each appointment, as the report alone is not sufficient. If another physician or facility has your films, please pick them up prior to your appointment with us. It’s usually not advisable to rely on courier services. Without films, your evaluation may be rescheduled.
The Federal Healthcare Insurance Portability and Accountability Act outlines the way your medical information is used. You can view a description of your rights at any time. You’ll be asked to sign an acknowledgment that you have been provided a copy of these rights.
We want to provide your medication refills as efficiently and safely as possible. Please give us two business days notice for refill requests and be aware that we may need to speak with you before authorizing a prescription. If you receive pain medication from us, we request that you use only one pharmacy and that you do not obtain narcotic prescriptions from any other physician at the same time.
